From the personal experience eleven years ago.
I married American citizen in Ukrane. After that we had to go to Poland to apply for mine (spouse) and my daughter's visas. Then, he went back to the United States. My daughter and I had to return to Ukraine and wait for three months until we got a notification that we can come to recieve our visas. We took another trip to Poland, went through complete physical exam by polish doctors appointed by American Embassy there, then had another interview in American Embassy before our visas had been approved.
Another option was to apply for visas in Moskow, which as we had been told, would be longer - up to six months. We would be much better off to go to Moskow as we have been there many times before and have had many relatives and friends there to stay with. I would strongly advise this second option for russian and ukrainian women because the trips to Poland were very costly for us. Futhermore, my daughter and I had some really scary experience in Poland - we and other people were robbed at gun and knife point by men in masks at night while we were staying at the hotel. Those men treatened to take my then seventeen years old daughter with them. By God's grace and kindness, we were not physically harmed or killed.
I'm not sure whether the whole process is easier or more compicated now.
Hope, its helps,
God bless

Wouldn't it be different if a US citizen went over seas and got married. How would you bring your new spouse home then?
